Indiana state rules on filing out incident reports in childcare centers when a child is hurt?

A friend of mine has a son who was playing on the playground at the childcare center where he goes to and fell off the playground set. He broke his arm and no incident report was ever filed. He was hospitalized for a day and now has to have xray’s every few days. The center has made no attempt to try to pay his bills we are guessing since he is on medicaid. His mother works at the center and is very upset at how they are going about all of this. Isn’t there a law that injuries HAVE to be documented and the parent sign? Another problem is that the teacher caring for her child didn’t follow procedure and call 911 but picked him up and carried him through the playground through the building and to his mother with his arm visibly broken
